    Position Overview

    Primary Responsibilities

    Clinical Services
    • Perform basic nursing assessments in clinic and community settings

    MNT Nutrionist I

    MNT Nutritionist I

    Full-Time Position
    Location: Paducah-McCracken County Health Center
    Pay Range: $21.52 – $26.75/hour

    Position Overview

    Under close supervision, performs moderate to complex nutrition case management for medically high-risk individuals. Provides physician-prescribed dietary counseling and Medical Nutrition Therapy (MNT) services in clinic and community settings. Reports to the Nutrition Services Supervisor or Director of Nutrition.

    Minimum Requirements
    Licensure (Required):
    • Registered Dietitian (RD) licensed by the Kentucky State Board of Licensure and Certification (KRS 310.021); or
    • Certified Nutritionist (CN) licensed by the Kentucky State Board of Licensure and Certification (KRS 310.031).

    No prior experience required.

    Essential Duties & Responsibilities
    • Assess nutrition status and develop individualized nutrition and MNT care plans.
    • Provide individual and group nutrition counseling.
    • Implement physician-prescribed dietary regimens.
    • Interpret nutrition research and provide consultation to healthcare staff.
    • Participate in on-the-job training and competency development activities.
    • Maintain documentation in accordance with agency policy and WIC & Nutrition Manual guidelines.

    Position Overview

    As a HANDS Ongoing Home Visitor, you will work directly with first-time parents and families, beginning in the prenatal period and continuing through the child’s second birthday. By providing regular home visits, you will build trusting relationships, support families in developing parenting skills, and connect them with vital health and community resources.

    This is a full-time, entry-level role designed for compassionate individuals who want to empower families, promote early childhood development, and make a lasting impact in the community. 

    Primary Responsibilities

    Family Engagement & Support

    • Conduct initial and ongoing home visits with eligible first-time parents and families.
    • Establish rapport and provide education on child development, parenting, nutrition, and health services.
    • Encourage parents to take an active role in accessing resources and making informed decisions for their child’s future.
    • Assist families in securing prenatal care, pediatric services, and referrals to supportive agencies.
    • Provide guidance on coping skills, safety, and early childhood milestones. 

    Program Implementation & Documentation

    • Maintain accurate case records, visit logs, and reports in compliance with HANDS program standards.

    • Protect the confidentiality of program participants and information exchanged with service providers.

    Minimum Qualifications

    Education

    • High School diploma or GED required. 

    Special Requirements

    • Must be at least 18 years of age.
    • Must have telephone accessibility, reliable transportation, and a valid driver’s license.
    • Must successfully pass a criminal background check and drug screening.
    • Completion of HANDS Core Training, wrap-around topical training, and CPR certification within 6 months of hire.
    • Ability to navigate within the community to locate participants and resources.

    Position Overview

    As a Public Health Dentist, you will play a key role in the Tooth Transit in-school dental program, delivering restorative care directly to students in grades K–12 across the Purchase District. Services are provided in a state-of-the-art Dental Mobile Unit in partnership with our public health dental hygiene team. This position combines clinical practice with community impact, allowing you to improve access to dental care and help establish a dental home for children in our region.

    This is a non-exempt, hourly contracted role ideal for dentists seeking work-life balance, meaningful community impact, and a mission-driven environment.

    Primary Responsibilities

    Patient Care

    Conduct routine dental examinations and screenings for students.

    Employee Requirements
